This area contains links to all necessary forms and applications for participation in the Slice of Shoreview.  Under most headings, you will find them in several formats:

• Online Form - Fill in the information, click the Submit button and it's sent to the appropriate committee
  
  
• PDF format - You will need the free Adobe Reader software installed on your computer to use this format.  Download the form and save it to your hard drive.  From there you can open the form on your computer, fill in the information, then save it and email it to the Committee. Or you can print it out, fill in the information by hand and mail it in.  
  
  
• Microsoft Excel format - Download the form, save it to your hard drive, type in the information and email it back to us.  You need Microsoft Excel software installed on your computer to use this format.  Or you can print it out, fill in the information by hand and mail it in.
